We are seeking a dedicated RN with Long Term Care experience to join our team at The Gardens of Cedar Rapids!

This position is responsible for assisting the Director of Nursing (DON) with planning, coordinating and managing the nursing department, along with completing the MDS assessments for the building. MDS training can be provided if needed. The MDS Coordinator is responsible for completion of the MDS 3.0. and should have the ability to communicate effectively and advocate for resident centered/directed care to team members, families, physicians, and allied health professionals. Salary is negotiable based on experience.

Responsibilities:

• Create and maintain appropriate care plans for each resident
• Complete bedside assessments per MDS guidelines
• Create a schedule for MDS assessments that ensures timely completion
• Check provider orders in the EHR system to ensure accuracy and completeness of documentation
• Assist with facility admissions and discharges as requested by the Director of Nursing
• Assist with nursing care on the unit as needed or requested, including assistance at meal times
• Work with the Director of Nursing and Restorative Nurse to ensure the highest quality of care and compliance with federal and state regulation
• Assist in insurance and chart reviews for skilled residents
• Additional duties as assigned by the Director of Nursing or Administrator
• Participate in a paid on-call nurse manager rotation every 6th weekend.

Qualifications:

• Must be a Registered Nurse from an accredited college, with current state license without encumbrance
• Responsible for maintaining professional licensure and CEU requirements.
• Current CPR certification required.
• 1- 2 years supervisory experience in a geriatric setting is preferred but not required

On-Call:

There will be expected rotating on-call during the week (1 day per week) and on- call every 5th weekend. You will also cover on-call for the DON when she is out of office.

The Gardens of Cedar Rapids is a locally owned and managed senior community that opened in August 2017. Our campus includes 30 apartment assisted living, 12 unit assisted living memory care, and 40 bed skilled nursing facility.
